If you need to withdraw your student from YES Prep Northwest, please use these steps for guidance.

Step 1 - Download and complete this document (Leaver Tracking Document). Complete first half of document ONLY.

Step 2 - Once the document (Leaver Tracking Document) is completed, email it to our Registrar (Karina.Carcamo@yesprep.org) with a copy of the guardians ID

Step 3 - Download and give this document (VOE) to the school the student is being transferred to (the new school will need to complete and return to YES Prep Northwest by faxing or emailing our Registrar)
